At Preferred Home Care of Florida, we partner with physicians to provide home health services for patients suffering from heart conditions.
In our CHF program, we provide patients with the tools to enable & empower them to take charge of their own CHF treatments as independently as possible.
How it Works
- Nursing Services, Physical Therapy, Occupational Therapy and Social Work
- Tools to monitor vital signs such as blood pressure, heart rate and oxygen saturation
- Monitoring daily weight by providing a free scale, education and weight log to trigger potential MD protocol for diuretics
- Medication management
- Diet and nutritional education
- An individualized therapeutic daily exercise routine to improve strength, endurance and mobility
- Monitoring vital signs and heart failure symptoms
